Perry Belmont House Foundation Donation Form The Perry Belmont House Foundation is a 501(c)(3) tax-deductible Foundation. The Foundation was formed specifically to receive donations for the restoration and preservation of the Perry Belmont House (the OES International Headquarters Building) located in Washington, D.C., the grounds on which the Building rests and the contents of the Building. Donations to the Foundation allows persons or organizations to take a personal tax deduction for any and all donations made. Please make checks payable to “Perry Belmont House Foundation.” All donors will receive a donation acknowledgement to include with their tax returns. Preservation and Restoration Projects Form 1618 Special Projects Donation Form 1618 Special Projects Lapel Pin The International Headquarters 1618 Preservation and Restoration Projects is a 501(c)(10) Project of the General Grand Chapter for the purpose of refurbishment and repairs of special projects at the International Headquarters. Depending on the size of donation, contributions to the 1618 Preservation and Restoration Projects can earn the donor a beautiful lapel pin, followed by five (5) ribbons with additional donations.
